Label Order & Database Instructions
These instructions cover the Label order form and the page used specify the
mailing list categories and provide the mailing list addresses.
LABEL ORDER FORM - Field-by-Field Information
Contact Information:

Name
|
Enter the first and last name of the
individual that we are to contact if there are questions
concerning your materials or order.

Email Address
|
Provide your Email address if we are to
contact you via Email. This
is also needed if we are to send verification of when we receive
your materials and when we ship your order
|
|
Daytime Phone
|
This is the telephone number we are to call
with questions between 9am and 5pm (your local time based upon
the ship to address). If
you do not want us to contact you during this time… leave it
blank.

Evening Phone
|
This is the telephone number we are to call
with questions between 5pm and 9pm (your local time based upon
the ship to address). If
you do not want us to contact you during this time… leave it
blank.

Billing Address:
|
Name
|
Enter the first and last name of the
individual that we are to bill.

|
Address Line 1
|
Provide the first line of the billing
address.
|
|
Address Line 2
|
If necessary provide the second line of
your billing address.
|
|
City
|
Provide the name of your city.
|
|
State/Province
|
Provide the abbreviation for your state or
province.
|
|
Zip/Postal Code
|
Provide the zip code or postal code used to
deliver your mail.

|
Country
|
Provide the country if it is outside of
USA.

Ship To Address (if different):
|
Name
|
Enter the first and last name of the
individual that we are to ship the Twin-Springs products and
return your materials to.

Address Line 1
|
Provide the first line of their mailing
address.

Address Line 2
|
If necessary provide the second line of
their mailing address.
|
|
City
|
Provide the name of their city.
|
|
State/Province
|
Provide the abbreviation for their state or
province.
|
|
Zip/Postal Code
|
Provide the their your mail.
|
|
Country
|
Provide the country if it is outside of
USA.
|
|
Phone Number
|
Provide their telephone number in case
we, or the shipper, needs to contact them.

Payment Information:
|
NOTE:
|
This information is only necessary if you
are paying by American Express, Discover Card, MasterCard, or Visa.
We need this information to properly charge your account
when we ship your order.

Name on Card
|
Provide the full name that is imprinted on
the account. This
may be different from the contact and shipping names.
|
|
Account Nbr
|
Provide the account number present on the
credit card.
|
|
Card Member ID
|
This is a security measure that insures
that the person providing the account number physically has the credit card.
American Express = the 4
pre-printed (non-embossed) digits above the account number on either the
right or left edge.
Discover Card = the 3 digits
immediately following the card number on the back in reverse indent printing
in the signature panel.
MasterCard = the 3 digits
immediately following either the last 4 digits of the account number or the
full account number on the back in reverse indent printing in the signature
panel.
Visa = the 3 digits immediately
following the full account number on the back in reverse indent printing in
the signature panel. |

Expire (m/d/y)
|
Indicate the Month, Day and Year that the
credit card expires.
|
|
Type of Card
|
Indicate which of the credit cards
that the account number is for.
|
|
Signature
|
Have the cardholder sign the order form to
confirm that it is acceptable for the charges to be made on
their card.
|
Label Order Information:
|
Label Type
|
This specifies which type of label is to
be used when we print the labels.
Matte White = This is a label
that is solid white and the envelope is covered with the label.
Clear =
This is a label that is clear and the label almost disappears when it is put
on the envelope or postcard.

Addr ID or All
|
This specifies if a single address label
is to be printed or all labels matching the category criteria are to be
printed.
Address ID =
This is a unique value assigned to this address. It is provided to you
on the master list that is sent with each set of mailing list updates.
All = This
indicates that all of the addresses that match the category value (see
below) are to be printed.

Nbr of Copies
|
This is the number of labels to be
printed for this specific address or all addresses that match the
specified category values.
|
|
Image ID
|
This is either the Image Id from the
Twin-Springs Icon web pages, or is an image that you are providing to us.
If no value is specified then there is
no image to the left of the address information on the label.
|
|
Match Category 1
|
This is the first of three optional
categories that the "All" addresses are checked against.
If left blank then there is no category
matching 1 value for this address. |
|
Match Value 1
|
This is the value that an address must
have in the Match Category 1 field in order to be printed.
|
|
Match Category 2
|
This is the second of three optional
categories that the "All" addresses are checked against.
If left blank then there is no category
matching 2 value for this address. |
|
Match Value 2
|
This is the value that an address must
have in the Match Category 2 field in order to be printed.
|
|
Match Category 3
|
This is the third of three optional
categories that the "All" addresses are checked against.
If left blank then there is no category
matching 3 value for this address. |

Match Value 3
|
This is the value that an address must
have in the Match Category 3 field in order to be printed.

NOTE:
If there are multiple matching category values then they
ALL MUST MATCH before an address is included. If you want multiple sets of
labels printed, then submit multiple sets of instructions each with a single

Initial Database Setup:
NOTE: The following
information is only established once. If a category is used it cannot be
changed. Blank categories can be set up at any time.

Category 1
|
This establishes the first of six
possible categories that may be used to select addresses to be printed.
Name - This is the name that is
used to refer to the first category. Examples include "Officers",
"Christmas", "Inactive", etc.
Valid Values - This specifies
valid values that may be used for this category. Normal values include
"Y", "N", or a specific value. |

This establishes the second of six
possible categories that may be used to select addresses to be printed.
Name - This is the name that is
used to refer to the second category. Examples include "Officers",
"Christmas", "Inactive", etc.

Category 3
|
This establishes the third of six
possible categories that may be used to select addresses to be printed.

Category 4
|
This establishes the fourth of six
possible categories that may be used to select addresses to be printed.
Name - This is the name that is
used to refer to the fourth category. Examples include "Officers",
"Christmas", "Inactive", etc.

valid values that may be used for this category. Normal values include
"Y", "N", or a specific value. |

Category 5
|
This establishes the fifth of six
possible categories that may be used to select addresses to be printed.
Name - This is the name that is
used to refer to the fifth category. Examples include "Officers",
"Christmas", "Inactive", etc.

|
This establishes the sixth of six
possible categories that may be used to select addresses to be printed.
Name - This is the name that is
used to refer to the sixth category. Examples include "Officers",
"Christmas", "Inactive", etc.

valid values that may be used for this category. Normal values include
"Y", "N", or a specific value. |

NOTE: The following
information is provided for each address. Updates can be submitted by
email, or by sending us a marked up list.

Enter the first and last name of the
individual or an organization.

|
Provide the first line of their mailing
address.

If necessary provide the second line of
their mailing address.
